Patient Resources
Patient Process
-
Book an appointment with me online. Or you can email, call, or text for more information. Someone will reach out to you within 48 hours.
In-office visits and telemedicine video visits are offered.
-
Patients are responsible for collecting previous medical records and other diagnostic records prior to meeting. I will not contact other offices, pharmacies, or diagnostic centers for patient records. This includes but not limited to any lab work within the past 1-3 years, relevant imaging, specialists and primary visit notes, and hospital and procedure records. Having records available allows better decision making and reduces appointment cost.
-
Additional diagnostics and records may be needed in between your initial new patient visit and follow-up visit. Patients are responsible for the cost and completion of these tests. Access Labs offers comprehensive tests from $110-$350.
-
After I have received all the necessary paperwork and diagnostics, a follow-up visit will be scheduled to go over treatment options, management plans, and recommendations. Visits typically last from 45-90 minutes. You will receive a customized care plan.
-
Patients that have signed up for monthly memberships or health coaching will have visits or follow-up communication every 30 days.
If you received compounded or other prescription medication, your medication will be refilled at that time, each month.
If you are an already established patient, a discounted rate is available for 3 or more visits
